Patient reports no history of this inMedicationsthe past.Fluoxetine, lisdexamfetamine, and Denies History of blood clots ormultivitamin family history of clotting or bleedingROSdisorders. Patient is not on bloodReview of systems: Positive for nausea, thinners. Patient denies being onvomiting, diarrhea, and fatigue. OCPsNegative for chest pain, fever/chills, Denies chest pain, fevers, orabdominal pain.abdominal pain.She was seen at this ED 3 daysPhysical exam: ago for 3-week history of sinusTemp: 37.2 C (99 F) HR: 101 BPMpressure and sinusitis associatedConstitutional: Alert, Answering with intermittent nausea, vomiting,questions appropriately, No Acute diarrhea, generalized weakness,distress, speaking in full sentences, no malaise, dizziness, and fatigue forincreased work of breathing GLU: 126, NA:137, K: 4.0, CL: 103, CO2: which she was treated with 2 full HENT: normocephalic, mucous23, AG:11, BUN: 19, CR 1.91 up from 1.69,courses of antibiotics and steroids.membranes are moist CAL: 8.4, Protein 6.2, Albumin 3.2, Alk During this ED visit, she was told she CARD: normal rate and rhythm,Phos 58, AST 16, ALT 5has mild anemia (Hb: 9.3) and AKInormal heart sounds DDIMER 3.03(Cr:1.69). Patient passed PO challenge PULM: normal effort, course breathand was discharged with PCP follow- sounds UA: Moderate Luk Est, Nitrite neg, Protein up.
