b'PACEP SCIENTIFIC ASSEMBLY 2024 CPC REVIEWThe Resolution:CPC CASE STUDY Syncope & Encephalopathy 44-year-old female presented to the ER after a syncopal episode, seizure-like activity andRESULTSloss of consciousnessTest to order: CT Angiography:Type A aortic dissection extending from the aortic root through thoracic and abdominal aorta into the bilateral common iliac arteries and external left iliac artery The dissection also affected right brachiocephalic artery, base of left common carotid artery, left subclavian artery, celiac artery, SMA and bilateral kidneys.Patient course: Patient was intubated, and esmolol, propofol and fentanyl infusions were started. Patient went to the OR to have aorta repaired but after multiple complications, the family decided to pursue comfort measures.Relevance to EM:This case highlights an uncommon presentation of a common ED diagnosis that carries a high morbidity and mortality for patients.
