Patient course: The patient was started on IV ceftriaxone, vancomycin and metronidazole and transferred to CHOP. The patient was taken to the OR for a frontal craniotomy for abscess. ENT performed a bilateral frontal endoscopic sinusotomy (FESS). He was discharged a few days later with no neurologic sequelae. Relevance to EM: This can occur after frontal bacterial sinusitis. The frontal sinuses are not present at birth and there is pneumatization and vascular development of the sinuses during childhood. The sinuses are not completely formed until adolescence. This diagnosis requires surgical intervention. Seizures may occur as a sequelae. Broad spectrum antibiotics are recommended to cover multiple organisms.
